The Alphabet Soup of Orthobiologics: PRP, PPP, BMAC, and MFAT

If you’ve been exploring treatment options for joint pain, tendon issues, or muscle injuries, you’ve probably come across terms like PRP, PPP, BMAC, and MFAT. It can be overwhelming, but these are simply different ways to use your blood, bone marrow, or fat tissue which helps your body heal naturally.

PRP — Platelet-Rich Plasma

PRP starts with a quick blood draw. The sample is spun in a centrifuge to separate the platelets, the small but powerful cells packed with growth factors that help reduce inflammation and support tissue repair. Think of PRP as your body’s own biologic “booster shot” for healing.

Common uses: Chronic tendon pain (like tennis elbow or jumper’s knee), mild joint arthritis, or post-procedure recovery.

PPP — Platelet-Poor Plasma

PPP is the lighter layer left over after PRP is separated. It’s lower in platelets but higher in plasma proteins that can calm inflammation and promote smooth muscle recovery. PPP helps the muscle heal more evenly and can reduce scarring, it’s the calming, smoothing side of the biologic family.

Common uses: Muscle injuries such as hamstring or calf strains.

BMAC — Bone Marrow Aspirate Concentrate

BMAC involves taking a small sample of bone marrow (usually from the back of the hip) under local anesthesia. It contains a mix of platelets, growth factors, and mesenchymal stromal cells (MSC’s) that can encourage tissue repair and reduce inflammation. BMAC is a step up in biologic strength, still all-natural, still your cells.

Common uses: More advanced joint damage, cartilage injury, or cases where prior treatments haven’t provided lasting relief.

MFAT — Micro-Fragmented Adipose Tissue

MFAT is made from a small sample of your fat tissue, gently processed to preserve its structure and cell population. Fat is rich in supportive and signaling cells (MSC’s) that send anti-inflammatory messages and promote tissue health. Think of MFAT as a biologic cushion, it helps both soften the impact and calm inflammation.

Common uses: Moderate to severe arthritis, chronic tendon or ligament issues, or painful joints that need extra cushioning.
